Children’s Hospital Orange County (CHOC) is conducting a community health needs assessment (CHNA) in Orange County. Your opinions and experiences are an important part of understanding the health concerns and needs in Orange County. Information from this survey will be used to inform CHOCs Community Benefit Implementation Strategy to advance community health and well-being.

This survey wants to hear your thoughts about health in Orange County and what life is like for kids and families. CHOC will use the answers to learn what health problems families face and what affects their health. This will help CHOC make a plan to improve health and wellness for kids and focus on the most important needs.
This survey will be open through May 12, 2025. It will take approximately 10 minutes for you to complete. It is available in nine languages – English, Spanish, Arabic, Farsi, Simplified Chinese, Traditional Chinese, Korean, Russian, and Vietnamese. Filling out the survey is your choice – it is completely voluntary. All answers are anonymous and not connected to your name.
There are no right or wrong answers. It’s your experience that matters! We encourage you to share this survey link with others in your community, so we can have the full picture of health needs in our communities.
